Simply complete the enrollment form and submit it by mail, email to Center Director or fax it to 410-676-9003 or in person and we will contact you to finalize your enrollment!
Before you begin be sure you have the following:
- Your child's most recent immunization records (Request from Doctor)
- The names and phone numbers for emergency contacts
If you have any problems using the enrollment form, please contact your Center Director or e-mail us at enroll@cclc.com.

- Applications are accepted year round on a first come, first served basis.
- Send Medical Immunization Records with your application for each child.
- Please return your application as soon as possible to secure slot availability.
- $50 Registration Fee required to complete admissions application.
